Basic errors killing 1000 NHS patients a month a study has revealed

The largest and most detailed survey into hospital deaths has revealed that almost 12,000 patients are needlessly dying every year as a result of poor patient care.
The researchers from The London School of Hygiene and Tropical Medicine based the study on 1,000 deaths at 10 NHS trusts during 2009. The study revealed that basic errors were made in more than one in 10 cases, leading to 5.2% of deaths, which was the equivalent of nearly 12,000 preventable deaths in hospitals in England every year.
The research published in the British Medical Journal's Quality and Safety publication found that errors occurred when hospital staff made an incorrect diagnosis, prescribed the wrong drugs, failed to monitor a patient's condition or react when a patient deteriorated. Errors in omission were more frequent than active mistakes.
The majority of patients who died were elderly suffering with multiple health conditions, but the study found that some patients whose deaths were preventable were aged in their 30s and 40s.

More supervision by senior doctors is required

The study concluded that more supervision by senior doctors is required to ensure patients are appropriately assessed and communication with GP's and social services takes place.
Helen Hogan, who led the study, said: "We found medical staff were not doing the basics well enough - monitoring blood pressure and kidney function, for example. They were also not assessing patients holistically early enough in their admission so they didn't miss any underlying condition. And they were not checking side-effects... before prescribing drugs."
Hospitals must learn from careful analysis of preventable deaths and make every effort to avoid [them]."

Imagine that a federal agency wanted to hurt America's working-class families on purpose. How would it inflict maximum damage?
It might start by aggressively marketing homeownership to marginal borrowers. It would tell them that bad credit scores aren't a problem. It would push them into homes they can't afford, saddle them with loans that barely build equity and provide no incentives for fiscal discipline. And when many of these homes go underwater and into foreclosure, it would leave families in financial ruin.
In short, such an agency would follow the Federal Housing Administration playbook.
That's a shame, because Republicans and Democrats alike rightly applaud the FHA's mission to provide responsible mortgage credit to low- and moderate-income Americans and first-time home buyers. But all too often, the FHA turns the American dream into a nightmare, setting up failure for the very families and neighborhoods its mission is to help.
This is not an isolated problem. A new study I completed at the American Enterprise Institute identified no fewer than 9,000 lower-income ZIP Codes where the projected foreclosure rate on loans insured by the FHA in fiscal years 2009 and 2010 is more than 10%. Overall, 1 in 7 families in these ZIP Codes stands to lose their home and their savings. Many areas had failure rates of 20%, 30% and even higher.
FHA policies are creating a real-life "tale of two cities," with the worst of times concentrated in working-class neighborhoods. In Chicago, the five ZIP Codes with the highest projected failure rate ranged from 35% to 73%, while the five lowest stood at just 0% to 4%. The comparable figures for Los Angeles/Riverside/San Bernardino are 13% to 17% for the five ZIP Codes with the highest failure rate, while the five lowest were all at 0%.
Remember, these loans were written well after the housing collapse. Today, the FHA's risky underwriting policies are backfiring in dramatic fashion in cities across America. Even in 2012, 40% of the FHA's loans are subprime — having a credit score below 660 or a debt-to-income ratio of 50% or more. To put this in perspective, the median FICO score for all individuals in the U.S. is 720, and the foreclosure risk on FHA loans increases substantially once the debt ratio exceeds 35%. When combined with minimal down payments and a 30-year term that builds equity slowly, the result is mortgage malpractice.
The FHA doesn't need to give up its mission. But it does need to acknowledge the harm its programs have caused. And it needs to follow a few simple principles that will stop setting up working families to fail.
First, end the practice of knowingly lending to people who cannot afford to repay their loans. The FHA uses its pricing advantages and lending policies to entice many low- and moderate-income families to take out irresponsible loans. Congress and the FHA refuse to stop this financing of failure because of special-interest groups. Start with the National Assn. of Realtors, which always supports looser standards because each marginal buyer represents another home sale, no matter how risky. Add community advocacy groups that promote "flexible" lending standards, which really mean risky loans with high default rates. Finally, the FHA focuses only on national averages while ignoring that the subprime loans it guarantees inevitably become highly concentrated in limited geographical areas.
To stop this harmful lending, the FHA should aim to cut its failure rate roughly in half, setting a maximum foreclosure rate of 10% on the loans it insures with an average foreclosure rate of 5%.
Second, help borrowers become owners, not debtors. The FHA nudges families onto a tightrope with no safety net, leaving them a broken water heater away from failure. Low-credit-score borrowers are eight to 14 times more likely to fail than a slightly less stretched borrower with the median credit score. Helping more borrowers take on 20-year, instead of 30-year, loans could cut their chance of losing a home 40%.
Third, concentrate on families that truly need help purchasing their first home. In recent years, the FHA has strayed far from its original mission, instead serving wealthier and wealthier home buyers as it quadrupled its insurance portfolio to $1.1 trillion. In fiscal year 2011, 54% of the FHA's dollar volume went to finance homes that were greater than 125% of an area's median house price. Since the FHA's mission is to help low- and moderate-income home buyers, the homes it finances should cost less than the median. Additionally, first-time home buyers should be limited to below-median incomes and repeat home buyers to incomes less than 80% of the median.
Fourth, step back from markets that can be better served by private lenders and insurers. The FHA has advantages that allow it to offer much lower rates than the private sector. Unless these advantages are narrowly targeted, they lead to unfair and dangerous competition with the prime and subprime private sector and political interference. The FHA should reduce its market share from today's 30% to a healthier 10% to 15%.
Taxpayers also have cause for concern. While recent data about sales volume and construction starts are encouraging, the same government mortgage complex that precipitated the crisis is still distorting the market. The FHA is just one part of that complex, and it's in deep denial. Were it a private mortgage insurer, regulators would shut it down for having a current net worth of negative $25 billion. It hasn't kept its own house in order, and as my research shows, it's continuing to push low- and moderate-income borrowers to make big, bad bets on homes they can't afford, with painful consequences.
Pursuing the American dream starts with a reality check. When Americans are given honest information about prices and risks, we make smart choices. But when the government tries to convince us that price and risk don't apply, hope turns to hopelessness.

While
it may not be a surprise that the Republicans are preparing to yield on
their vow to oppose tax hikes, it should raise investor concerns the
world over that an upcoming budget agreement will likely involve a
Congressional surrender of its authority to set the federal debt
ceiling. In exchange for this, it appears that the Republicans have
simply done nothing to halt, or even curb, the dangerous federal
spending trajectories or the current drift towards greater state control
of the economy. President Obama has politically outmaneuvered the
Republicans, even going as far as evoking the Newtown massacre as a
reason for quickly concluding a deal. As a result, it is likely that the
GOP will bear the blame for any breakdown in fiscal cliff negotiations.
They could wear such an outcome as a badge of honor, but nothing
indicates that they have the political courage to do so.


Given the drift in Washington, those who had hoped for a
significant improvement in the United States fiscal prospects will have
nothing but lumps of coal in their Christmas stockings. At the same
time, the Fed continues to hold its foot to the floor with increased
quantitative easing that will pump massive liquidity into the system
(see Peter Schiff's latest commentary for more on this). The combination
can lead to stagflation and a debased U.S. dollar.



Republican House Speaker John Boehner has indicated a willingness to
accept new taxes of $1 trillion, despite his party's pledge to halt tax
increases. Diverting a trillion dollars out of the wealth-generating
private sector into the government's coffers can only hurt corporate
investment, particularly the venture capital sector that is so vital to
the flow of new ideas and technology that stand at the core of America's
highly profitable international technological lead.



But far more momentous than his capitulation on taxes is his
agreement to pass debt limit power to the president. This decision
removes any chance that the United States will ever do anything
meaningful to address its fiscal imbalances.



Following the establishment of the Federal Reserve, the House of
Representatives' approval of the Federal debt limit was needed to
restrict a profligate government from endangering the credit of the
United States and encumbering future generations with an unbearable
level of debt service. But in order to pander to voters, both
Republicans and Democrats have paid little heed to this important duty
and have rubber-stamped continued increases in debt. In recent years,
when Federal debt began exceeding GDP, Republicans finally withheld the
stamp in order to force a real budget discussion. But a combination of
their own hypocrisy and political mishandling has rendered those
attempts futile.



In the meantime, both parties have avoided making any meaningful cuts
in government spending and have instead offered accounting gimmickry as
a poor substitute. Neither party appears willing to risk votes in order
to curb the profligate and economically suicidal growth in out of
control entitlement expenditures.



Last week, The Economist magazine published a list of 80 countries,
ranked in order of quality of life as determined by such elements as
economic well-being, intrusive government and crime. The United States
has now fallen to sixteenth position (with Switzerland in first
position). The combination of $1 trillion of new taxes and the
likelihood of increased financial and commercial regulation will likely
push the U.S. further down the list.



It is time for politicians, and particularly Republicans, to put the
nation before their individual careers. Should they fail in this great
patriotic call, they will be sentencing their fellow Americans to a
future of falling living standards, currency decline and poverty.



But while the US struggles with its political and economic morass,
other regions of the world do present more promising opportunities for
investors.

Americans are approaching an auspicious anniversary: it has been two hundred years since the first known appearance of "Uncle Sam" as an initialistic embodiment of the United States. The earliest example of "Uncle Sam" was found in the December 23, 1812 issue of the Bennington (Vermont) News-Letter. But another town not too far from Bennington — Troy, New York — has maintained that it is the true birthplace of Uncle Sam.
The Bennington example was discovered in a newspaper database several years ago by inveterate word sleuth Barry Popik. It appears in a letter to the editor from an anonymous conscript in the War of 1812, who is less than sanguine about the war effort.
Before Popik's discovery, the earliest known example came from The Troy (NY) Post the following year, on September 7, 1813, also from a critic of the government's actions in the war. The writer likens the new expression "Uncle Sam" to the established nickname for England, "John Bull": "This cant name for our government has got almost as current as 'John Bull.' The letters U. S. on the government waggons, &c., are supposed to have given rise to it."
In Troy, a more elaborate origin story for "Uncle Sam" emerged a couple of decades after the war. As first told in the New York Gazette in 1830, the story involves a meatpacker from Troy named Samuel Wilson who procured a contract to supply meat to the U.S. Army at the outbreak of the war. Because his barrels of meat were stamped with "U.S.," a joke began to circulate that this abbreviation really stood for "Uncle Sam," i.e., Sam Wilson.
Over the course of the 19th century, the lore of Samuel Wilson as the source of "Uncle Sam" became further entrenched. A writer named Albert Matthews published a monograph in 1908 that looked at all the known evidence and determined that it was highly unlikely that Wilson was the true originator of "Uncle Sam." Though "Uncle Sam" started to appear in Troy newspapers in 1813 as a playful personification of the U.S. and its government, no mention is made of Wilson until much later. The December 1812 Bennington example would seem to put the Wilson theory to rest, especially because it predates the meat contract that Wilson arranged with the Army the following year.
But Troy has firmly held on to the Samuel Wilson tale, much as another upstate New York town, Cooperstown, clings to its questionable reputation as the birthplace of baseball. The citizens of Troy even managed to get a Congressional resolution passed in 1961 saluting "Uncle Sam Wilson of Troy, New York, as the progenitor of America's National symbol of Uncle Sam."
Further digging in the databases may fill out the picture of how "Uncle Sam" first spread. Interestingly, "Uncle Sam" may have been a stock character in New England even before it came to stand for the United States. In Howe's Almanac for the Year of Our Lord, 1808 (published by John Howe in Greenwich, Mass. in 1807), the following advice is given on the calendar for the days of July 12-14: "Be diligent, and you will get done haying before Uncle Sam."
This generic "Uncle Sam" appears in Howe's Almanac in subsequent years. The 1809 almanac says, "Be at work early, and do not let Uncle Sam get done haying first." In 1810, the advice is, "Hoe your corn the third time or you will not have so much as Uncle Sam." And finally in 1811, a note for September 20-21 reads, "It is about time for Uncle Sam to finish haying." When war broke out in 1812, "Uncle Sam" took on a new meaning, originally used in a derisive manner by those opposed to the government's maneuvers. Eventually, of course, "Uncle Sam" would be embraced as a patriotic icon. Let's remember the bicentennial of his true origins, and not the mythmaking of Troy.
